Report an Incident
Injuries, illness, vehicle events, near misses, property damage, security concerns and safeguarding concerns are all reported the same day. Every report is reviewed by the Incident Reporting Coordinator.

Give as much detail as you can. If someone is in immediate danger, call for emergency help first, then file this report.
Reports are stored securely and shared only with the leaders responsible for reviewing them. Please do not include medical records or other sensitive documents in this form — the Safety & Security team will request them if needed.
